Step 1: Read District Use of Facilities Policy

Step 2: Apply to Become a Requester below

Step 3: Wait for your approval email

Step 4: Submit the application a minimum of seven working days prior to the activity.  Large events require a minimum of 30 days advance notice

Step 5: Provide Current Insurance Liability Form before your first use date.

Note: Facility use will not be allowed without current liability insurance on file with the School District of Superior. All individual/group/organization are required to have a certificate of insurance with a minimum of $500,000 liability coverage that identifies the School District of Superior as the certificate holder for all persons and activity/events. Contact Ray Kosey (715)394-8720 ext 41137 if you'd like to check on the status of your liability insurance or if you have questions about facility use.

Step 6: Once approved as a requester, log in to your account to place Facilities requests.
Step 7: Wait for email of approval/denial of your facilities request.  To check on the status of your request, log in to your account and click on the request history tab.
Step 8: When a request has been approved, an electronic contract will be sent to the requester. Sign the electronic contract with the pin you created in your account.

District Facilities Use Requirements

  1. Each group requesting the use of District facilities must use the services of a District custodian/engineer and must pay for such services.
  2. Each group requesting the use of District facilities must indicate an individual, satisfactory to the administrator in charge of the building, who will serve in a supervisory capacity during the use of the District facilities.
  3. If the supervisor, as designated above, is not satisfactory to the administrator in charge, the administrator shall appoint a supervisor and the cost shall be charged to the sponsoring group.
  4. Supervisors are responsible for the enforcement of all rules and procedures regarding the use of the District facilities.
  5. The custodian on duty is directed not to open the facility until the supervisor for the sponsoring group is on duty.
  6. A certified lifeguard must be on duty at all times to supervise when the rock climbing wall at Superior Middle School is occupied.
  7. The list is not exclusive, as other factors may require further requirements.
